Error: System.ListException: Row with duplicate Name at index 1

If you receive the following errors when you try to send a fax manually (Figure 1) and when you click the “Fax Configuration” tab (Figure 2) this is likely caused by duplicate custom field settings in the Fax Configuration tab (Figure 3). Solution is to delete one of the duplicate custom settings.
